Capt. Rita Burris said the accident occurred at Royal Food Products Inc. about 7 a.m. Wednesday on Indianapolis' southeast side.
Burris said the worker apparently drove onto a section of concrete floor that wasn't reinforced and it collapsed beneath the forklift, sending it plunging 14 feet into the basement. She said the worker and machine were trapped in debris.
Royal Foods, 2322 E. Minnesota St., makes salad dressing, sauces and other products. The plant was shut down for the day after the accident.
Burris said the worker was a man in his 50s. Officials didn't immediately release his name.
