  1. There’s a lot to unpack here. I’m not sure how CATO posed the questions, but the vast majority of progressives in the US are social democrats – as in the social democracies in Europe that provide universal health to people, support day care, have low or tuition-free education and, most importantly, are capitalist countries. US neoliberalism, on the other hand, is an extreme form of capitalism that has resulted in 60% of the working population living paycheck to paycheck and created, due to a wholly regressive tax policy, a billionaire class and overly corporate-friendly campaign finance laws that had led to these interests having grossly outsized influence on the levels of government. In terms of Medicare about 95% of payments were made properly in 2024 and improper payments have steadily declined. The Kaiser Family Fund asserts that most fraud in Medicaid is committed by providers. And in 2024 95% of payments were made properly. Most improper payments in Medicare were from lack of information. Sounds like the providers to me. And, of course, the Trump Administration has inflated the improper payment amounts. If the author is really interested in fraud, abuse, and waste, she should take a look at the DOD and weapons manufacturers. In a 2025 report, the US GAO asserts that the DOD is potentially significantly underreporting fraud by contractors – which the DOD reported was merely $11 billion over 7 years.
